Summary of Major Responsibilities

The Population Health Manager is directly responsible for supporting the quality assurance and program leaders within payers, employers, onsite-near site clinics, health systems and capitated clinical practice sites in achievement of improving quality metrics associated with cancer screening and care management. The individual in this position will be responsible for maximizing screening with Cologuard, through innovative and effective quality payor programs, leveraging education and delivery of quality improvement best practices.

 

The Population Health Manager will engage in designing programs in support of closing care gaps in screening, working with health insurers as they engage their members through alternative care models, employers/purchasers, retail, and health systems.  The Manager will educate and support alignment across payer clinical transformation teams, population health, quality nurses and staff in key medical groups, payers, ACOs, health systems, retail, and employer groups on the value proposition of Exact Sciences products as well as best practices to achieve population health objectives and improve population level screening and re-screening rates for Cologuard and its pipeline of products.

Essential Duties and Responsibilities

  • Support implementation of population health management programs and initiatives with customers and accounts to address closing gaps in care with a focusing interacting with clinical transformation Managers/medical quality teams and provider relations in payers, employers, retail, and health systems.
  • Coordinate primarily with the Market Access, Employer/Retail, as well as Health Systems teams to develop account and channel specific strategic plans to ensure effective utilization of branded and unbranded tools that include Exact Sciences products as part of the suite of evidence-based strategies included in screening programs designed to close colorectal cancer screening gaps.
  • Coordinate primarily with the Market Access, Employer/Retail, as well as Health Systems teams and the Exact Sciences Laboratories to support data transfer and data sharing to enable success of population health and quality improvement initiatives.
  • Design and conduct educational programs for payer and health system medical quality teams, to assist in their overall knowledge of preventive screening standards for colorectal cancer as well as brand specific product knowledge.  This includes all aspects of implementing Cologuard within the health system or payer customer target accounts for ordering, compliance monitoring and patient follow up.
  • Determine best practices utilized by payer, health system and medical group practices for closing screening gaps and implement a strategic plan to incorporate Cologuard into the programs by both the accounts and their supporting vendors.  Ensure Cologuard is part of the armamentarium utilized for closing screening gaps in appropriate patients.
  • Communicate account and program activities and provide status updates routinely to key internal stakeholders working cross-functionally with sales, marketing and medical affairs.
  • Develop and maintain an advanced perspective of the evolving clinical and healthcare landscape.
  • Utilize advanced health information technology (HIT) understanding to help assist customers and internal colleagues in leveraging data analytics to address population health issues.
  • Provide support to customers in gaining access to health economics and outcomes data and assist customers in the use of this information with the goal of optimizing quality improvement and population management initiatives within their organizations and geographies.
  • Maintain excellent working knowledge of disease area(s) and Exact Sciences products in order to provide context for functional and technical excellence in collaboration with other commercial colleagues/groups and medical colleagues within Exact as well as priority customers.
  • Ensure that customer insights are shared with relevant leadership to assist in informing future field-based population health strategies and tactics.
  • Engage with key stakeholders and influencers within the market, such as regional and national Quality Improvement Organizations, Advocacy Groups, Coalitions and other medical/healthcare associations.
  • Effective English communication skills, both written and verbal.

Qualifications

Minimum Qualifications

  • Bachelor's degree in Life Sciences, Nursing, Public Health, Business; Healthcare Administration or related discipline.
  • 7+ years of experience in any of the following areas: payer, contracting, coverage, quality improvement, and population health with large national payers; sales leadership in biotech, device or diagnostic laboratory companies or a background in quality improvement and population health in a health plan, health system, or laboratory, biotech, device or pharma company.
  • Basic computer skills to include Internet navigation, Email usage, and word processing.
  • Proficient in Microsoft Office to include Excel, Word and PowerPoint.
  • Demonstrated ability to perform the Essential Duties of the position with or without accommodation.
  • Authorization to work in the United States without sponsorship.

Preferred Qualifications

  • Master’s degree in Life Sciences, Nursing, Public Health, Business; Healthcare Administration or related discipline.
  • Registered Nurse (RN) or PharmD.
  • Of the advanced degrees listed above, prefer Masters in Public Health, Health Administration or Population Health.
  • Experience with presenting clinical and health economic information to groups and individuals.
  • Background working on quality initiatives in a payer, health system or large group practice.
  • Cross-functional and project management skills and experience.
  • In depth understanding of population health initiatives in preventive medicine and operational needs for health systems, ACOs, medical groups, payers, and employers/purchasers.
  • Experience with value propositions and driving pull-through at the payer, provider, and health system level.
  • Background working on quality initiatives in a commercial organization (pharma, biotechnology, diagnostic laboratory, or medical device), employer, health plan, health system or large group practice.
